Initial Consultation

In order to get the most out of the initial consultation with your attorney and save both time and expense, fill out our custody intake form and gather the following documents for the attorney's review:

1. Tax returns for the past five years;
2. Income information for you and your spouse, including paystubs, 1099's and W-2's for the past year; and
3. Insurance records including policies, applications, and invoices.
